The Online Pre-Admission Process with ReadyPath
The first step in getting you ready for surgery is ReadyPath’s online pre-admission process. This is a confidential health history questionnaire to identify specific things about you that help us plan ahead for your care. Please be prepared to tell us about your current medications (including dosages), supplements, over the counter medications, health conditions, allergies and other important health information. Once you are ready to click the “Get Started” button, you will be taken through a questionnaire with the following sections:
- Health history
- Medications
- Infectious disease screening
- Social history
Please complete the online part of the pre-admission process promptly to ensure that you can go ahead with your surgery as planned.
After you answer the questionnaire, you will be asked to submit your transportation arrangements, as well as your phone call and appointment preferences. It is important to plan and confirm your ride home from surgery in advance. If you have a family member or friend in mind, contact them promptly to make arrangements once you know your discharge time. If you do not have a family member or friend who can give you a ride home, the Transportation section in ReadyPath’s pre-admission process can help you connect with medical transportation services in your area.
Follow-up Phone Call
Once you have submitted the questionnaire and your preferences, your provider will review your information promptly and follow up with a phone call. During this call, the nurse will ask you follow up questions based on your questionnaire answers, provide additional instructions and may schedule your COVID test. You can ask the nurse any questions you have during this call. Be prepared to write down what medications should be taken on the day of surgery. Be sure to ask your physician if you should stop taking any of your medications or supplements prior to surgery. Our registered nurse will answer your questions and review any special instructions with you. For example, you may be asked to have diagnostic testing done, such as an EKG, blood draw or X-ray before the surgery.
Make sure to regularly check the email you signed up with for updates from your provider. We will let you know when your questionnaire is received, if your surgery is confirmed and other important details about your surgery.
COVID Test
As part of the pre-admission process, every patient must be tested for COVID 72-96 hours prior to their surgery. About a week before your surgery, a COVID test scheduler will contact you with information about your scheduled COVID test. Be sure to read through the Surgery and the COVID-19 Pandemic topic in the Important Information section so you are informed about any COVID-related updates.
